Complete an Electronic Master Promissory Note (EMPN) if you are a first-time borrower of a Federal Direct Subsidized or a Federal Direct Unsubsidized Loan at DePaul. 

Direct Parent Loan (PLUS) Electronic Master Promissory Note (EMPN)

Complete an Electronic Master Promissory Note (EMPN) if this is your first time borrowing a Direct Parent Loan (PLUS) at DePaul or if a different parent is now borrowing the PLUS.

Direct Loan Entrance Counseling

Complete the Entrance Counseling requirement for all Direct Stafford Loans.

Direct Parent Loan (PLUS) Application Supplement Forms

Print and complete the PLUS Supplement form and submit to DePaul.

Federal Perkins Loan Requirements

If you are a first-time Perkins Loan borrower, complete your Perkins Electronic Master Promissory Note (EMPN) and Entrance Counseling.

If you are borrowing an additional Perkins Loan at DePaul, then you must complete a Perkins ESIGN/Reaffirmation for each year you borrow the Perkins Loan.

Federal TEACH Grant

If you are an Undergraduate student enrolled in an eligible program of study and would like to request consideration for the Federal TEACH Grant, you must complete and submit the Request for TEACH Grant Consideration Form for 2009-2010 to the Office of Financial Aid.
