Illinois Monetary Award Program (MAP) Recipients: If you enroll in 15 credit hours or more throughout the academic year, you will receive one-third of your Illinois State Grant during each quarter. If you enroll in less than 15 credit hours or for only part of the academic year, you will receive a prorated award in accordance with ISAC rules.

Federal Loan Forms & Links
Change of Loan Request: If you would like to cancel, reduce or reinstate
a financial aid award that you have been offered, you may do so by
completing and submitting this form to the Financial Aid office.
If your loan has been partially or entirely disbursed, please contact
our office prior to submitting the Change of Loan Request Form. Once
there has been a disbursement made on your loan, our office is limited
in its ability to change that loan amount. In addition, if our office
is able to change a loan amount that has already disbursed to your
student account, please keep in mind that we are removing loan funds
from your student account; therefore the removal of your loan
disbursement from your student account may result in a balance due to
the university.

Stafford Electronic Direct Loan Master Promissory Note (EMPN): If this is the first-time you're borrowing a federal loan at DePaul you are required to complete a Master Promissory Note and Entrance Interview. To complete the MPN electronically, please visit the Direct Loan MPN website. (Don't forget to also complete your Entrance Interview, using the link below.)
Stafford Electronic Entrance Interview: To complete the Entrance Interview electronically, please visit the Direct Loan Entrance Counseling website. If you need a copy of the Entrance Interview Rights and Responsibilities worksheet, please download the form. (Don't forget to also complete your EMPN, using the link above.)
Parent Loan for Undergraduate Students, "PLUS" Supplement: This supplement is necessary to process your initial PLUS Application, or your request for additional PLUS funds. If this is the first-time you're borrowing a PLUS at DePaul (or a different parent is now borrowing the PLUS) you are also required to complete an Electronic Master Promissory Note. To complete the PLUS Master Promissory Note electronically, visit the PLUS MPN website.

Loan Exit Interview: If you borrowed a federal loan and are graduating from or leaving DePaul, you are required to complete loan exit counseling. By logging on to the Exit Interview site and completing the on-line interview you'll satisfy your exit requirements.
